Identification

All patients must present a valid, non-expired state-issued Driver’s License, State issued ID card, passport, or
Passport ID card.

Privacy Policy

This practice complies with HIPAA and is committed to protecting the privacy and confidentiality of patient health information. Protected Health Information (PHI) is used and disclosed only as permitted or required by law, or with written patient authorization.

While safeguards are in place, no electronic communication or storage system can be guaranteed to be completely secure. By using this website or electronic communication, you acknowledge these inherent risks.

Patients have rights regarding their health information, including access and requests for copies or amendments as allowed by law.

Consent Policy

Before starting our telehealth services, we obtain informed consent, which outlines the nature of therapy, including psychotherapy and medication management, potential benefits and risks, your rights, and your ability to withdraw consent at any time. This process is crucial, especially in the context of child mental health and psychiatric evaluations.

Controlled Substances Policy

Novara Psychiatry may prescribe controlled substances for patients diagnosed with anxiety, insomnia, or ADHD as part of their comprehensive medication management strategy. All patients (and guardians, if applicable) must sign a controlled substance agreement before starting treatment. Please be advised: 


  • Random drug screens or medication counts may occur at any time. 
  • Use no more than two pharmacies for prescriptions. 
  • A positive test for illicit drugs, non-prescribed medications, or missed doses may lead to the discontinuation of controlled substances. If needed to prevent withdrawal, a short supply (no more than 30 days) may be provided. The patient will no longer be eligible for future controlled substance prescriptions from Novara Psychiatry PLLC and will need to seek another provider for these medications. 
  • Non-controlled medications can continue at Novara Psychiatry PLLC, where we also offer psychiatric evaluations, psychotherapy, and telehealth services, ensuring a holistic approach to mental healthcare services.

Prescription Policy

Our office will provide refills and renew prescriptions ONLY during appointments. This will reduce errors, improves patient safety, and ensures appropriate follow-up. 

Cancellation and No Shows

To ensure we provide quality care to clients receiving mental health services, medication management, psychiatric evaluations, and psychotherapy, we kindly ask that you provide advance notice for any cancellations or rescheduled appointments. Medication management appointments require at least 24 hours’ notice, and initial sessions/psychotherapy sessions require at least 48 hours’ notice. Please note that late cancellations may be subject to a fee of up to 50% of the session rate, and no-shows may be charged up to 100% of the session rate.

Discontinuation of Services

Patients may terminate care at any time for any reason.

Novara Psychiatry PLLC may also terminate the provider–patient relationship at its discretion with written notice in cases including, but not limited to, abusive or threatening behavior, illegal activity on premises, inappropriate communication, repeated nonadherence to treatment or clinic policies (including three or more missed or late-canceled appointments), or failure to meet financial obligations.

Termination of care does not relieve the patient of any outstanding balances owed for services rendered.

Forms and Documentation Services

There is a fee for completion of paperwork, forms, and provider letters, including FMLA, disability, and work or school accommodations.

FMLA and disability paperwork are not completed during the initial visit. In general, patients must be established in care for at least six months before these requests will be considered.

Completion of all documentation is at the provider’s discretion and based on clinical judgment and medical necessity. Submission of forms does not guarantee completion or approval, and processing times may vary.
